What is a Serious Case Review?

Birmingham Safeguarding Children Board is responsible for carrying out Serious Case Reviews where a child has died and abuse or neglect has been confirmed or suspected.  In certain circumstances, a serious case review may also take place when a child/ young person has been seriously harmed.

When the case review is complete, an Executive Summary is produced outlining the key findings (personal details are anonymised).

BSCB supports the implementation of learning from serious case update training and the production of anonymised case studies.

Purpose of a Serious Case Review

The purpose of the Serious Case Review is as outlined in Chapter 8 (8.3) of the Working Together To Safeguard Children 2006 guidance, namely:

  1. Establish whether there are lessons to be learned from the case about the way in which local professionals and organisations work together to safeguard and promote the welfare of children.
  2. Identify clear what those lessons are, how they will be acted upon, and what is expected to change as a result.
  3. As a consequence, to improve inter-agency working and better safeguard and promote the welfare of children.
